New Patient Appointments

To request a new patient appointment, please complete the New Patient Request form. After submitted our office will do our best to contact you within 1 - 2 business days to schedule an appointment, excluding weekends, holidays, and scheduled office closures. ​

Online Scheduling - Established Patients Only

Online scheduling is a convenient option for established patients, designed to help manage ongoing health and wellness, whether for a follow-up visit, hospital follow up, same-day, annual wellness exam, telehealth, or other routine appointment with your Pacific Noble Primary Care provider. 
 

If you don't see an available appointment time that fits your schedule, please call our office directly

Your care team may be able to:

  • Offer an appointment time not listed in the online scheduling tool

  • Add you to our cancellation wait list in case a sooner appointment becomes available

We are committed to helping our patients access timely care and will do our best to see you as soon as possible.


Please note: If you are new patient interested in establishing care, please complete the New Patient Request form so we can begin processing your request and scheduling your first visit. 

Confirming an Appointment

To help ensure timely access to care, we ask that all appointments be confirmed and any requested electronic forms submitted 24 hours prior to a scheduled visit

  • If we do not receive confirmation, your appointment may be rescheduled. 

  • If you are unable to complete the electronic forms in advance, please contact our office so we can allocate time to assist you during your visit. 

Canceling or Rescheduling an Appointment

We value and respect our patients, clinicians, and staffs time. If you need to cancel or reschedule an appointment, please Contact Us as soon as possible so we can offer the appointment time to another patient in need of care. To cancel or reschedule, you may contact us by phone or email. 

  • We understand that unexpected circumstances can and do arise. As a courtesy, established patients are allowed one missed appointment without a no-show fee. After that, a $50 no-show fee will apply for any missed appointments or same-day cancellations without 24-hour prior notice. 

  • For new patients, we kindly ask that you provide at least 24 hours' notice if you need to cancel or reschedule your first appointment. New patient appointments missed without 24-hour notice will be subject to a $50 no-show fee, and excluded from the one-time established patient missed appointment courtesy. 

 

Please note: Three consecutive no-shows may result in patient dismissal from the clinic, as we strive to ensure continuity and access to care for all patients.
