New Patient Appointments
To request a new patient appointment, please complete the New Patient Request form. After submitted our office will do our best to contact you within 1 - 2 business days to schedule an appointment, excluding weekends, holidays, and scheduled office closures.
Confirming an Appointment
To help ensure timely access to care, we ask that all appointments be confirmed and any requested electronic forms submitted 24 hours prior to a scheduled visit.

If we do not receive confirmation, your appointment may be rescheduled.

If you are unable to complete the electronic forms in advance, please contact our office so we can allocate time to assist you during your visit.
Canceling or Rescheduling an Appointment
We value and respect our patients, clinicians, and staffs time. If you need to cancel or reschedule an appointment, please Contact Us as soon as possible so we can offer the appointment time to another patient in need of care. To cancel or reschedule, you may contact us by email or phone.

We understand that unexpected circumstances can and do arise. As a courtesy, we allow one missed appointment without a no-show fee. After that, a $50 no-show fee will apply for any missed appointments without 24-hour prior notice.

Three consecutive no-shows may result in possible patient dismissal from the clinic to ensure continuity of care for all patients.
